“Fludrocortisone 100 mcg: Uses, Dosage, Side Effects & Warnings”

What Is Fludrocortisone?

Fludrocortisone acetate is a synthetic corticosteroid with strong mineralocorticoid properties and mild glucocorticoid effects.

🔹 Primary Uses:

ConditionRole of Fludrocortisone
Primary Adrenal Insufficiency (e.g., Addison’s Disease)Replaces mineralocorticoid activity (aldosterone)
Congenital Adrenal Hyperplasia (CAH)Helps maintain sodium and potassium balance
Orthostatic Hypotension (off-label)Increases blood volume and blood pressure
Salt-losing conditionsHelps retain sodium and fluid

Mechanism of Action

Fludrocortisone binds to mineralocorticoid receptors, primarily in the kidneys, to:

  • Increase sodium reabsorption in the renal tubules
  • Promote water retention
  • Increase potassium and hydrogen ion excretion
  • Help maintain blood pressure and blood volume

Dosage & Administration

🔹Typical Adult Dose:

  • 100 mcg (0.1 mg) orally once daily
  • Dosage may be adjusted based on electrolyte levels, blood pressure, and clinical response

🔹Pediatric Use:

  • Common in congenital adrenal hyperplasia; dose is weight-based and monitored closely

🔹Monitoring:

  • Serum sodium, potassium, blood pressure, and weight should be regularly monitored
  • Adjust dose based on signs of fluid overload or hypokalemia

 Side Effects

🔹Common Side Effects:

  • Headache
  • Swelling (edema)
  • High blood pressure
  • Dizziness
  • Weight gain
  • Muscle weakness

🔹Serious or Long-Term Effects:

  • Hypokalemia (low potassium)
  • Hypernatremia (high sodium)
  • Fluid overload
  • Heart failure or hypertension
  • Gastrointestinal ulcers
  • Glaucoma or cataracts (rare)

Warnings & Precautions

  • Avoid abrupt discontinuation after long-term use—taper if stopping
  • Use cautiously in patients with:
    • Heart disease or hypertension
    • Kidney impairment
    • Edema or fluid retention
    • Infections (can suppress immune response)

 Special Considerations

  • Patients on fludrocortisone often also take hydrocortisone to replace glucocorticoid activity
  • May require salt supplementation early in treatment or in high-temperature environments
